YOUR PATIENT CENTERED MEDICAL HOME
Thank you for choosing Southpoint Family Medicine as your new medical home provider. Our goal is to provide you excellent care in a welcoming environment.
A medical home is an approach to providing care for all. It creates both better healthcare and lower costs. With your medical home, you are joining a team that provides evidence-based, centralized, comprehensive, and coordinated primary care. There is a partnership between patients, their personal physicians, the medical home team, as well as the patients’ families.
In a medical home, the goal is that the practice be organized around the patient and that patients have access to personalized, coordinated, and comprehensive primary care. This is achieved with open communication, respect, and shared decision-making.
It is essential for all new patients to provide a complete medical history when establishing care with us. It is important to share any information about care received outside of our office such as hospitals and specialists.
Please complete the new patient packet to review with your provider at Southpoint Family Medicine.
