A medical home is an approach to providing care for all. It creates both better healthcare and lower costs.  With your medical home, you are joining a team that provides evidence-based, centralized, comprehensive, and coordinated primary care. There is a partnership between patients, their personal physicians, the medical home team, as well as the patients’ families.

In a medical home, the goal is that the practice be organized around the patient and that patients have access to personalized, coordinated, and comprehensive primary care. This is achieved with open communication, respect, and shared decision-making.  

It is essential for all new patients to provide a complete medical history when establishing care with us.  It is important to share any information about care received outside of our office such as hospitals and specialists.

• Bring your list of questions and medications to each appointment

• Bring your photo ID, insurance card, and co-pay to every visit

• Call our office before going to the emergency room or hospital. We can get you into the office on the same day if needed and we always have a provider available on call after hours

• Inform other providers you may see that Southpoint Family Medicine is your medical home and ask them to share with us information regarding the care they provided you.
